Conceived in 2015 by Alzheimer's Orange County's CEO, Jim McAleer, the first OCSPA Leadership Council meeting took place in August 2016. Since that time, significant momentum has occurred, contributing to the creation of a robust short-term Strategic Plan. Strategy: Bring together cities, the county, non -profits, foundations, and corporate entities to evaluate opportunities and create a structure to address opportunities. Problem: Within the next 15 years, Orange County's senior population will grow by 63%, with Orange County being among the fastest growing aging counties in America. While there have been multiple efforts to gather data across sectors, this will be the first strategic plan since 1998 to identify the areas of greatest need and coordinate multiple resources to address those needs. Vision: Orange County is a place that creatively nurtures, encourages, and supports every person as they progress through each stage of life. Leadership Council: The Orange County Strategic Plan for Aging is guided by an ongoing planning body comprised of community leaders: Orange County Supervisor Lisa Bartlett, Alzheimer's Orange County, CalOptima, Abrazar, Inc., SeniorServ, Council on Aging — Southern California, OC Health Care Agency, OC Office on Aging, OC Transportation Authority, Irvine Health Foundation, Senior Citizens Advisory Council, Jamboree Housing, OC United Way, UC Irvine Health, Easterseals Southern California, Orange County Community Foundation, and the Asian American Senior Citizens Services Center. The Leadership Council is chaired by Jim McAleer, CEO, Alzheimer's Orange County. 15 Participating Cities: Anaheim, Brea, Buena Park, Costa Mesa, Fountain Valley, Garden Grove, Irvine, La Habra, Laguna Niguel, Laguna Woods, Mission Viejo, Newport Beach, Santa Ana, Tustin, and Villa Park. 18 -Month Wrap Up Senior Nutrition and Food Security GOAL: Increase food security for Orange County's older adults age 60+ by enrolling new participants in CalFresh. Establish new Commodity Supplemental Food Program and Senior Grocery distribution sites for food insecure older adults. Status of Goal - COMPLETE There have been 545 new adults over age 60 enrolled into CalFresh since July 1, 2017. The subcommittee has surpassed the 18 month goal of 425 new enrollees. Seven new Senior Grocery sites and fifteen new Park -It Market food distribution sites have been opened since July 1, 2017. The subcommittee has surpassed the 18 month goal of opening eight new sites. The OC Food Help website is now live that includes an interactive food map with all food distribution sites and an online appointment system for CalFresh. Technology GOAL: Enhance and accelerate the capacity of the digital platform, IrisOC, to create a local digital hub to distribute healthy aging information, provide connections to senior -focused community resources and people, and acquire pilot users of the platform. Status of Goal - COMPLETE 0 A beta version of the IrisOC platform launched in October 2018 ORANGE COUNTY �. STRATEGIC G P N N 18 -month Committee and Subcommittee Goal Wrap Up July 1, 2017 — December 31, 2018 Transportation GOAL: Conduct a data review to identify top transportation needs and explore innovative options. Present the findings at an OCSPA Summit. Status of Goal - COMPLETE • The Orange County Referral Guide for Older Roadway Users transportation guide was created and released. The guide contains resources to assist older adults and their family members with questions and concerns related to safe driving. Topics covered include: driving assessments, refresher classes, adaptive equipment for vehicles, and transportation alternatives. • A phone survey was conducted to determine the available services and barriers each city experiences when it comes to senior transportation. Findings will contribute to the OCSPA Aging Report. Healthcare GOAL: Double the number of older adults who complete an evidence -based chronic disease and/or diabetes self- management education training workshop. Status of Goal - COMPLETE • The subcommittee's goal was to have 700 senior participants complete an evidence -based chronic disease self- management workshop. The subcommittee has surpassed the 18 month goal with 909 completers. • An informational flyer is available in English, Spanish, Vietnamese, Korean, and Mandarin. • Classes are posted on the California Healthier Living Website. Housing GOAL: Nonprofit housing developers and universities will work together to identify the disparity between older adult housing needs in Orange County and housing availability. Release study findings to community leaders at an OCSPA Summit. Status of Goal - COMPLETE • Contract staff from Cal State University Fullerton has completed the data analysis. All findings were shared with the research team working on the OCSPA Aging Report which is to be released in spring 2019. Social Engagement GOAL: Reinstitute the Friendly Visitor Program Coalition to support capacity building and increased information - sharing among the existing service programs, resulting in better services to isolated/homebound adults. Status of Goal - COMPLETE • The 1s' Heart to Heart Conference took place in February 2017 at Lakeview Senior Center. It was a successful event that drew 80 attendees representing 33 nonprofit and faith based organizations. This event gave attendees the opportunity to enhance and develop community and ministry -based visitation programs in Orange County. • A Friendly Visitor Toolkit was revealed at the Heart to Heart conference and over 50 Friendly Visitor Toolkits have been distributed to local programs and ministries to support capacity building, volunteer training and increased awareness of community resources. Quarterly Visitation Council meetings were formalized to bring together friendly visitor programs and visitation ministries to continue sharing best practices and resources with one another. The Subcommittee held 3 Visitation Council meetings bringing together a total of 90 attendees over the past 18 months. ORANGE COUNTY S0 RTEAIG P N N 18 -month Committee and Subcommittee Goal Wrap Up July 1, 2017 — December 31, 2018 Elder Abuse Awareness GOAL: Launch an education and awareness campaign regarding elder abuse awareness and prevention at two Orange County cities, targeting the general public, first responders, academic institutions, and mandated reporters. The goal is to have 500 Orange County residents attend a financial abuse awareness and prevention workshop. Status of Goal: - COMPLETE • The subcommittee's goal was to educate 500 people who have attended the workshop. They have surpassed the goal with educating 574 attendees. To date the trainers have taught 200 attendees from the general public, 215 professionals and 159 students. Other Updates OC Successful Aging Members of the committee have been working with AARP to identify and outreach to key cities regarding membership in AARP's Age Friendly Communities Network. OCSPA Aging Report The Orange County Community Foundation and the Irvine Health Foundation are funding an aging report based on OCSPA's priority areas. OCSPA has been working with UC Irvine Health Division of Gerontology & Geriatric Medicine and Cal State University Fullerton's Center for Successful Aging to gather existing and new data As of December 2018, all key data points have been summarized with key recommendations and are now in the editing and design phase. The goal is to have the report released at the Orange County Aging Services Collaborative Leadership Forum on Aging scheduled for April 2019. More details will be released soon. In most people with Alzheimer's, symptoms first appear in their mid -60s. Estimates vary, but experts suggest that more than 5 million Americans may have Alzheimer's. The terms "Alzheimer's disease" and "dementia" are often used interchangeably as many people believe that one means the other. Dementia is an umbrella term for a set of symptoms including impaired thinking and memory. Alzheimer's is the most common form of dementia. However, issues other than Alzheimer's can cause dementia, such as Vascular Dementia, Frontotemporal Dementia, Lewy Body, Huntington's Disease, Parkinson's Disease and Creutzfeldt-Jakob Disease. Cognitive Activity: Read books, play strategic games, learn new skills, keep learning Exercise & Physical Health: Exercise regularly, monitor blood pressure and sugar levels, get enough sleep, stop smoking, manage stress and visit your Social Engagement: Visit with family and friends, stay involved in the community, join a group or club, volunteer outside the home, take a class Diet & Nutrition: Eat vegetables, fruits, whole grains, nuts, lean meats, fish and poultry.
