NOTICE OF ACCEPTANCE OF
IRREVOCABLE OFFER OF DEDICATION
This Notice of Acceptance is in reference to that certain Irrevocable Offer of
Dedication made by Hoag Memorial Hospital Presbyterian, a California non-profit public benefit
corporation to the City of Newport Beach, a California municipal corporation of property located
in the City of Newport Beach, County of Orange, State as California described on Exhibit "A"
attached hereto (the "Property"), approved by the City Council of the City of Newport Beach
at their regular meeting held on October 10, 1994 and recorded on April 20, 1995 in the Official
Records of Orange County, California as Instrument No. 95-0165541.
The terms and conditions set forth in the Irrevocable Offer of Dedication have
been satisfied and the City wishes to record this Notice of Acceptance -of the Property in the
Official Records of Orange County, California.

This is to certify that the Property described on Exhibit "A" conveyed by the Irrevocable Offer
of Dedication from Hoag Memorial Hospital Presbyterian to the City of Newport Beach, is
hereby accepted by the undersigned officer or agent on behalf of the City of Newport Beach and
City consents to recordation thereof by its duly authorized officer.
