A client intake form is completed for each investigation or each Advanced Health Care Directive completed. This form requests information on payment source. Man residents are on SSI (Supplemental SeuritY Income) which automatically places them in low income status. All these residents will benefit by Ombudsman advocacy even if not personally counted statisticallyas unduplicated. With full funding, your Ombudsman will make over 150 unannounced facility Yosft in the fiscal year 2016-2017 to accomplish the above.
