The following documentation is required to complete the application: 1. Agreement. a. Please note, a bond is required prior to construction. b. Please note, a master license agreement amendment is required prior to construction in order to identify the specific site locations approved under the agreement. c. Please note, an encroachment permit is required prior to construction. 2. SCE Letter. Provide a letter from Southern California Edison authorizing the power supply for this site location. Provide the SCE power location and approved drawings from SCE as well. Please refer to the attached City issued Meter Pedestal Denial Letter needed. 3. Project Plans. See the attached plans from Real Property, Public Works, and Utilities for additional redlined comments. Please provide one full size (24” by 36”) set of plans and four reduced (11” x 17”) sets of plans that are revised to reflect the following information. Topographic Survey (Sheet C-1) a. Provide distance between pole and tree. Site Plan (Sheet A-1): b. Show all utilities. c. P I:\Users\PLN\Shared\PA's\PAs - 2019\PA2019-108\PA2019-108 Incomplete Ltr 2nd Submittal.docx Page 2 d. Dimensions provided do not match city records. See redlines. e. Add a note that “Avocado Avenue and Third Avenue are on the City’s street/alley-cut moratorium list. Trenching into said as part of this development will require extensive alley repair work”. Details of such repair work may be obtained from the Public Works Department. f. Keep conduits in side walk. g. Clarify who is performing work for fiber handhole. Elevations (Sheets A-2 and A-3): h. Specify the Avocado Avenue travel directions. i. Show all below grade utilities. j. Revise notes to be consistent with General Notes. Details k. Signage shall have anti-graffiti coating. l. Provide details for luminaire attachment Sheet E-1 (Electrical and Grounding) m. Where will the disconnect from existing power supply be located? Please revise all necessary drawings (including elevations) to clearly show where the disconnect will be located. n. #6 should read green. 4. Traffic Control Plan. Traffic Control Plans are required as part of the encroachment permit application. Either remove TCP from this plan set or clearly note that Traffic Plans are not to be used for construction and a new TCP will be submitted and approved as party of the encroachment permit application. 5. Electronic copy. All of the above submittal items are required to be submitted in electronic format (i.e., CD-ROM or PDF via email). Upon verification of completion, the application will be processed and scheduled for a Zoning Administrator Hearing.
