We are providing you with an Owner- uilder Acknowledgment and Information Verification Form to
make you aware of your responsibilities and possible risk you may incur by having this permit issued in
your name as the Owner -Builder.
We will not issue a building permit until you have read, initialed your understanding of each
provision, signed, and returned this form to us at our official address indicated. An agent of the
owner cannot execute this notice unless you, the property owner, obtain the prior approval of the
permitting authority.
OWNER'S ACKNOWLEDGMENT AND VERIFICATION OF INFORMATION
DIRECTIONS: Read and initial each statement below to signify you understand or verify this information.

1. I understand a frequent practice of unlicensed persons is to have the property owner obtain an
"Owner -Builder" building permit that erroneously implies that the property owner is providing his or her
own labor and material personally. 1, as an Owner -Builder, may be held liable and subject to serious
financial risk for any injuries sustained by an unlicensed person and his or her employees while working
on my property. My homeowner's insurance may not provide coverage for those injuries. I am willfully
acting as an Owner -Builder and am aware of the limits of my insurance coverage for injuries to workers

2. 1 understand building permits are not required to be signed by property owners unless they are
responsible for the construction and are not hiring a licensed Contractor to assume this responsibility.
%AP 3. 1 understand as an "Owner -Builder" I am the responsible party of record on the permit. I
understand that I may protect myself from potential financial risk by hiring a licensed Contractor and
having the permit filed in his or her name instead of my own.
6 _4. 1 understand Contractors are required by law to be licensed and bonded in California and to list
their license numbers on permits and contracts.

Contractors, and the total value of my construction is at least five hundred dollars ($500), including labor
and materials, I may be considered an "employee' under state and federal law.

state and federal government, withhold payroll taxes, provide workers' compensation disability insurance,
and contribute to unemployment compensation for each "employee." I also understand my failure to abide

owner and returned to the City of Newport Beach, Building Division, for issuing the permit.

presented when the permit is issued to verify the property owner's signature.

1, FORMATION
1.1 Name. The name of this limited liability company (the "Company") is
Mainland Investments, LLC.

Penn Street, Suite 205, Whittier, California, but it may be relocated by the Managers at any

designated office will be at 500 N. Rainbow Boulevard, Ste. 300, Las Vegas, Nevada, and the
name of its initial agent for service of process at that address will be Professional Legal

14.5 Additional Documents. Each Member must execute such additional
documents and take such actions as are reasonably requested by the Managers in order to
complete or confirm the transactions contemplated by this agreement.
14.6 Counterparts. This agreement may be executed in two or more counterparts,
which together will constitute one agreement.
14.7 Governing Law. This agreement will be governed by the law of the state in
which the articles of organization of the Company have been filed.
14.8 Severability. If any provision of this agreement is invalid or unenforceable,
this will not affect the remaining provisions.
14.9 Third -Party Beneficiaries. The provisions of this agreement are intended
solely for the benefit of the Members and Managers and create no rights or obligations
enforceable by any third parry, including creditors of the Company, except as otherwise
provided by applicable law.
4 14.10 Authority. Each individual executing this agreement on behalf of a
corporation or other entity warrants that he or she is authorized to do so and that this
agreement will constitute the legally binding obligation of the corporation or other entity that
the individual represents.
