The following documentation is required to complete the application: 1. Mailing Labels: Mailing labels must be submitted as both an Excel spreadsheet and Word document (Avery 5160 label format). Please see the attached Public Hearing Requirements for electronic formatting for the Excel and Word files. 2. Plans: A 3-foot-wide fire department access is required to rescue windows and exterior doors. The proposed plans indicate a 1.5-foot-wide separation between the existing single-family residence and proposed structure. Provide a full rear elevation of the existing single-family residence showing all windows and doors. Depending on the location of existing rescue windows and exterior doors, the proposed structure may require relocation to provide the 3-foot-wide fire department access between buildings. 3. Acoustical Study: Please provide an acoustical analysis report per the attached Acoustical Study Guidelines. Ensure an illustrative design concept for the proposed method of sound attenuation is provided. For your information: This project is pending review from the Building Department. The Building Department’s Project Review Request will be forwarded once available and any corrections, if any, must be addressed in order to complete the application. The soil vapor extraction and treatment system will consist of a 20’ x 12’ x 10’ treatment system building, underground pipe network (approximately 2,400 linear ft), and 13 extraction wells for soil gas remediation.
