`I conducted exterior inspections on 11/1/07, 11/10/07, and 11/15/07 of Fury Rok & Rol Sushi
Louge to answer the following:
1. Is there a valet service?
No. There is not a valet service.
2. Are they stacking cars. in the off-site lot?
No. There is no stacking of vehicles off -site.
3. Is the off -site lot being utilized by employees?
I did not witness any employees parking offsite and walking to Fury. I noticed certain
employees parking in the shared lot and walk into the back door of Fury. (photos attached)

At least one parking space is. "reserved" for Fury noted by a sign posted in the stall. (photos
attached)
4. Is there outdoor storage ?"
Yes. There is outdoor storage of boxes, trash cans, refuse bin, mop cart, blue 55g barrels,
brooms, dust pans, and crate(s). (photos attached)
5. Are they complying with the conditions related to trash?
Condition 34 of UP 2005 -018 requires periodic steam cleaning of dumpsters and dumpster
areas to be cleaned according to NBMC Section 14. This is requirement is on an as needed
basis. There are no findings for this condition.
Condition 35 of UP 2005 -018 requires all refuse dumpsters to be stored within a walled
enclosure. One dumpster is being stored outside of a trash enclosure. (photos attached)
6. Is there any unpermitted signage?
The permanent signage is permitted per the Planning Department. All permanent signage is `
considered change out with electrical previously permitted. There is temporary signage
which is not in compliance with NBMC 20.67.090. There are two temporary signs that are
oversized. There is a temporary sign permit for one of those signs, BN20070085. (photos (AA

1 conducted an inspection on 11/29/07 as a follow up .
This second inspection was to determine whether employees of Fury are adhering to the
prescribed Parking Agreement 86= 149743.
1 arrived at the parking structure located at 4141 Dove Street at 430pm on Thursday
11/29107. Accompanying me were Matt Cosylion and Sgt. Ron Vallercamp. We met at the
fifth level of the structure to watch the back entrance of the restaurant and the arrival of
employees. At no time did anyone park on the fifth level of the structure then enter Fury. I
witnessed employees walking to and from vehicles parked at the shared lot. Photographs
were taken of vehicles, some of which had previously been identified as employee vehicles
at an inspection dated 11/15/07 (photos attached).

It has been determined that Fury is niether adhering to the parking agreement nor utilizing
structure parking for employees.
