SUBJECT: Purchase of Chest Compression Devices (AutoPulses @)
RECOMMENDATION:
The City Council approve the purchase of six AutoPulses@ (chest compression devices)
for $92,484 from Zoll Medical Corporation.
Authorize budget amendment to transfer $92,484 from the General Fund
Unappropriated Paramedic Reserve to account 2340 -9300 to fund the total purchase
amount.
DISCUSSION:
The AutoPulse works as an automatic cardiac support pump, utilizing a wide
compression band that squeezes the entire chest. Currently, the Newport Beach Fire
Department (FD) does not possess any mechanical devices capable of providing chest
compressions for patients requiring cardiopulmonary resuscitation (CPR). The only
method utilized is manual chest compressions provided by FD personnel. The American
Heart Association's (AHA's) most recent guidelines strongly emphasize effective
compressions as the most essential aspect of successful resuscitation. They also
recommend minimizing interruptions of chest compressions and rotating compressors
every 2 minutes.
The direct benefits of the AutoPulse include the following: First, this device provides a
more effective alternative to manual chest compressions. Even when performed well,
manual chest compression only provide 10 -20% of normal blood flow to the heart and
30 -40% to the brain. AHA cites studies of chest compressions performed in the
prehospital care setting where 40% of the time chest compressions were of insufficient
depth. Other studies have shown rescuers who fatigue after only one minute of
performing chest compressions which further decreases its effectiveness. With the
AutoPulse, victims receive consistent, high - quality compressions that deliver improved
blood flow. The automatic feature of this device would also eliminate interruptions that

normally take place when rotating manual compressors or when moving a patient from
one location to another (i.e. down staircases or through confined areas).
Secondly, the AutoPulse will improve safety of FD personnel. With this device, our
personnel would not need to stand in the back of the ambulance during transport and
would have the opportunity to wear seatbelts. Performing CPR in the back of a moving
ambulance is not only dangerous to the rescuer, but also does not allow for consistent,
quality chest compressions for the patient.
Thirdly, the AutoPulse would allow better utilization of resources. Because cardiac
arrest calls are typically labor intensive and require much physical effort on the part of
FD personnel, a total of three fire apparatus are automatically dispatched. During
transport, extra personnel must ride along to assist the paramedics with critical tasks.
The AutoPulse would free up personnel that could be better utilized during the
resuscitation for other purposes or could be returned to a fire apparatus that is fully
staffed and available to handle other emergency calls for service.
We recently conducted a 2'/: month trial where the AutoPulse was used on 16 cardiac
arrest patients. The feedback obtained by FD personnel during the trial was positive and
did demonstrate that the device performed as expected. The AutoPulses will be placed
on all of the FD's Paramedic ambulances (three first line and two reserve) and one fire
engine housed at Station 8.
There are currently no other devices that can mirror the AutoPulse's unique mechanism
of action. Zoll Medical Corporation is the only company that manufactures and markets
the AutoPulse. Zoll announced recently that all existing quotes will expire September
27, 2008, after which time there will be a projected increase of $1500 per unit.
Environmental Review:
The Staff Report has no requirement for Environmental Review.
Funding Availability:
It will cost approximately $92,484, not including tax and shipping costs to purchase six
units. The Administrative Services Director recommends we fund the $92,484 from the
General Fund Unappropriated Paramedic Reserve.
