NOMINATION FORM
Leadership Tomorrow will honor two individuals at Celebrate Leadership Event in February. Two very
special awards will be presented: The Excellence in Community Leadership Award, which is open to
nominations from the general public, is presented to a person /persons who best represent Leadership in
our community, regardless of affiliation or position, and The Excellence in Leadership Award, is presented
to a person/persons who have been affiliated with Leadership Tomorrow and best represent the
characteristics of leadership for the past year.
The event will take place at Park Priv6 on Thursday, February 5, 2009 from 5:30 to 7:30 pm. To nominate
a person for either award, please submit the nomination information below. If you need more space to
write, please feel free to attach a separate page. The deadline to submit is December 8v', 2008.
