Become a member of the Grand Champions Club
by raising $1000 or the Champions Club by raising
$500. In addition to funding the care, support and
research efforts of the Alzheimer's Association,
members of the Champions Club receive the
following benefits:

• Champions Club commemorative medal
• Fundraising badge on your personal Walk Web page
• Thank you certificate recognition on the national
Walk thank you Web page
*All participants who raise the fundraising minimum of
$100 will receive an official Walk to End Alzheimer's t -shirt

The 2015 Walk to End Alzheimer's will take place on
three dates:

SATURDAY, NOVEMBER 7:

Huntington Beach, CA 92648
(intersection of Beach & PCH)
Complimentary transportation from First
Christian Church of Huntington Beach (1207 Main St.,
Huntington Beach, 92648) to the Walk location will be
provided. State Beach parking lot,
between 1st St. and Beach Blvd., is $15. Metered parking
is available along PCH & Beach Blvd.
SATURDAY, NOVEMBER 14:
2000 East Gene Autry Way, Anaheim, CA 92806
Please enterthe Stadium off of Orangewood
Ave or Gene Autry Way. Upon entry attendants will guide
you to complimentary parking.

We encourage all participants to fundraise $100 to
earn a 2015 event t -shirt. However, there is no fee
