Entity Address (if different from applicant): Name of Program or Service: Second Chance Orange County Type of Program or Service offered to Newport Beach residents: SCOC offers a one-on-one support program of services to recovered addicts/alcoholics designed to assist and encourage their return to productive lives through numerous specific support services aimed at employment, stable housing, and ongoing sobriety. Have you previously received City support for this or a similar;program or activity? YES NO 0 ❑ If yet, please list the programs(s), years) and amount(s)° CDBG 2018. We expect to receive $6000 this year. Number of Newport Beach residents participating or receiving services within past year. (Specify numbers by type of service.) We are presently supporting five NB residents with wrap-around services. Two are presently employed, two homeless clients are now housed, and four of the five are receiving ongoing mentoring and coaching. Percentage of this program devoted or.utilized by Newport Beach residents: We serve all of Orange Countyand focus on those communities that have provided us specific funding through CDBG or general fund contributions. Newport Beach is one of four OC cities where we have a focus, and our donated office space is.in the Melinda Hoag Family Health Center in NB. Quantifying a percentage of program services to a community Iis difficult when our organization serves clients who maybe homeless, and be referred from a specific city, but who will be housed in another city, .because that is where we can make a placement. In our focus cities like NB, we are careful to track residency, and we actively seek referrals from those cities who support us. Total annual fundraising/donations/sponsorships received by your organization: Our recent annual fundraiser generated more than $32,000. Donations come in all year long and in varied forms -- from volunteer hours worth several hundred thousand dollars to specific dollar amounts and donated items for our fundraiser. Our sponsorships are in the form of workplace partners -- businesses that work with us to employ our clients, which is impossible to quantify in dollar amounts. Brief description of program or activity for which you are seeking a grant: Our program will assess recovering addicts/alcoholics who display a commitment to ongoing sobriety and who desire a return to productive lives. We offer them ongoing support through employment training classes, by helping to fund their basic needs for shoes, clothing and other essentials, and by assessing their workplace readiness. We find them jobs, monitor their progress, and the goal is for each one to leave our program economically improved, working, in stable housing, sober and ready for next steps. Explain how your program or activity serves, involves or engages Newport Beach residents (150 - word maximum): Our program serves Newport Beach residents who are ready to return the community as contributing members. We assess their needs - physical, mental, work readiness, and housing, and we work to balance their interests with their training and their eventual work placement through our employment connections. They are coached and mentored at every step. Our program benefits the entire city, though we work directly with only a few, because each client we help reduces the city's response costs in policing, in emergency services, in hospitalization costs, and sometimes, sadly, even mortuary costs. Our clients stay out of NB jails and courtrooms, and stay off city streets where they are characterized as homeless vagrants. Our success -- and theirs -- is that they serve the city and their neighborhood as sober, tax -paying, productive people. The City of Newport Beach cares about these residents, and this grant is tangible evidence of their commitment to address community needs and issues. The request for City of Newport Beach support must include a list of other City Yes No entities, groups or programs (e.g., CDBG, Discretionary Funds, Special Event grants, etc.) from which the applicant is seeking financial support. Are you seeking support from other City entities? If you answered yes above, please list the entities or groups and the amount of financial support requested from each: We requested funds from Newport Beach CDBG and were awarded a $6000 grant. Amount of financial assistance received last year (FY 17-18) from the City of Newport Beach (if any): $ None Amount of financial assistance requested FY 18-19: We are requesting $8000 from this grant. A detailed description of how the FY 18-19 grant money would be spent: This grant will allow us to double our efforts in Newport Beach. Our clients come with built-in costs that vary from person to person. Some are homeless; they need clothing, shoes and haircuts. Most need job training so they are versed in customer service and what to expect in formal interviews. Some need to renew licenses, or get on payment plans to reduce debt. Nearly all need counseling, and all need to stay on their assigned treatment protocols. None of this is free. We monitor each one with the help of mentors and volunteers, and case manage according to accepted standards. This grant would allow us to serve more NB residents and underwrite these costs. Does.your organization work with minor children? Yes NO If yes, do you require background checks for employees and .volunteers? Y No fail ❑` Please explain: We have no full time employees except the director, but all part-time and as -needed help is vetted according to standard workplace personnel policies. Our volunteers are vetted for security and work history and sign confidential agreements regarding all clients.
