My name is Mr. Chuyen Ngo, I am the CEO of CDA Global Corporation, and the owner of the
property located at 63 Cape Andover. I would like to change the contractor of record on my
project from MD & Associates to The Michael Salvini Company as of March 27a' 2018. The
permit number is X2018-0206. I have attached documents stating that I am the owner of CDA
Global Corporation.

FIELD MEMORANDUM

Job Address: 63 Cape Andover, Newport Beach
Subject: Flag pole footing excavations- addition to existing residence
Observations
Date: June 15, 2018
Two flag pole footing excavations in conjunction with an addition to the upper level of the
existing residence have been constructed into firm, unyielding soils (engineered fill) that consists
of medium dense to dense, silty to clayey sand.
The approximate locations of observed footing excavations documented within the scope of the
field memorandum are shown on the attached portion of the Foundation Plan, Sheet S2.0 by
DCA Structural Engineering. (See reverse side.)
Conclusions
The footing excavations are acceptable, from an engineering geologic viewpoint, for
placement of steel and concrete.
Recommendations
Dimensions and details to be verified by others.
As a condition for use of this field memorandum, the property owners and builder shall accept
the risk of minor structural and cosmetic cracking between improvements supported by new
footings andjlagse supported by the existing foundation.
