Describe Inspection Made, including locations:

Observed the installation of threaded rods into the existing concrete using
Simpson xp epoxy, expiration 2020. Verified the hole depths and proper cleaning
with a nylon brush and compressed air prior to the epoxy. Per schedule and
details on the approved plan. Detail on S-4 / K
Locations:
1) Detail on 8-4 / K
A) Pile #1 and Pile #2 with (2) 5/8" threaded rods with min. 6" embed. Per Pile
2) S-4 / T = (N) 4x8 Ledger with 5/8" rods @ 16" o.c with min. 6" embed.
Work is complete for the areas listed.

To the best of my knowledge, the work inspected was in accordance with the Building Department
approved design drawings, specifications and applicable workmanship provisions of the IDC except as
noted above.

on Pg S-3 on approved plans. Numbered 1 & 2 for easy reference.
Piles numbered and listed on Pile Schedule Pg S-3. Detail V - Pile 1 & 2
3'x0" 14" Sq. T -Pile 30' long.
TX0" 14" Sq. T -Pile 30' long
Observed the placement of the (2) T -piles - minimum embedment of 15' met for
each pile.
Work is complete for each pile location.
