After the top of slab/floor elevation is verified to match the elevation specified on the approved drawings, the contractor and inspector can measure the height of the structure to the top of slab/floor to verity that it is equal or less than the dimension shown on building sections and elevations. This form must be filled out by a registered surveyor or civil engineer authorized to perfonu surveys. Grading and subgrade compaction of the building pad were generally accomplished as follows: (1) Prior to the grading, the subgrade was cleared of existing asphalt concrete, construction, and other debris, and was hauled off-site. (2) The building pad was excavated 2 -feet below bottom of the footings and extended 5 -feet outside building perimeter. (3) After the bottom was approved, it was scarified to an additional 6 -inches and compacted. (4) All recompacted backfill and new fill was moisture conditioned to near optimum, mixed, and compacted in approximate 6 inches thick lifts. (5) On-site granular materials were used for backfilling. (6) Fill compaction was accomplished with rubber wheel rolling compaction equipment. (7) The minimum degree of compaction obtained for all subgrade, backfill was at least ninety percent (90%) of the ASTM D-1557 Laboratory Test Standard. Field Density Test (ASTM D-6938) In-place, compacted fill densities were determined in accordance with current ASTM D- 6938 and ASTM D-1556 field test procedures. Scope of Work This report provides a summary of the field geotechnical inspections, observations and compaction testing provided by this office during the footings, asphalt concrete parking, subgrade preparation, aggregate base, asphalt concrete, utility trench backfills e.g., sewer, water lines, irrigation line, fire lanes, sidewalks, and miscellaneous subgrade within the existing Our Lady Queen of Angels Church & School facility located at 2046 Mar Vista Drive in the City of Newport Beach, California. Grade changes relative to original topography required to secure final subgrade elevation were negligible. The approximate limits of grading are presented on the Plot Plan and Compaction Test Location Maps (Plates B and C) in Appendix A. B. Earthwork Grading Procedures: Grading was performed during the period of February 7, 2020, through June 18, 2021. Gymnasium building location, grading limits and approximate test locations are provided on Plot Plan and Compaction Test Location Map, Plates B and C in Appendix A. In general, earthwork operation was accomplished as follows: Backfill compaction was accomplished by track rolling of earthwork grading machineries. 2 II Our Lady Queen of Angels Gymnasium — Final Grading Report Project No.: D-2569-02 Grading and subgrade compaction of the other development features were generally accomplished as follows: (1) Prior to the grading, the subgrade was cleared of construction and other debris, and was hauled off --site. (2) The subgrade of the pavement and concrete walkway areas were then scarified 12 -inches below finish subgrade and recompacted prior to placing aggregate base. (3) Footing excavations, subgrade, and aggregate base of slab on grade was inspected by a representative of this firm as this office was notified by the site superintendent. (4) All recompacted backfill and new fill was moisture conditioned to near optimum, mixed, and compacted in approximate 6 inches thick lifts. (5) On-site sandy materials and approved imported aggregate base were used for backfilling. (6) Fill compaction was accomplished with rubber wheel rolling, Wacker, and other compaction equipment. Aggregate base and asphalt concrete were compacted by rollers and Wacker. The sewer pipe was placed over 6 inches of clean sand bedding. Fire Line Fire line trench was backfilled with mix of on-site and imported material and was compacted to minimum 90 percent of ASTM D-1557 laboratory test standard. The field density tests (W-1 through W-8) are 'shown on Plate C and test 5 Our Lady Queen of Angels Gymnasium — Final Grading Report Project No.: D-2569-02 results are summarized in Table 4. Water line was generally 24 inches in depth. Bedding under the pipe consisted of very fine clean sand. Electrical Line Electrical line trenches were backfilled with mix of on-site and imported material and were compacted to minimum 90 percent of ASTM D-1557 laboratory standard. Some of the electrical trenches were 6" wide or less. No compaction test was taken for trenches with width of 6" or less. The locations of the field tests (E-1 through E-12) are shown on Plate C and test results are summarized in Table 5. Electric trench was 2 to 3 feet in depth and placed over native soil. Aggregate Base Aggregate Base locations are shown on Plate B and the test results (BI through B18) are tabulated on Table Six. Class II aggregate base was placed over subgrade, spread, and rolled obtaining minimum relative compaction of 95 percent of ASTM D-1557 laboratory standard. Asphalt Concrete Driveway areas were graded and paved as per plans. The specified pavement section was 4" AC over 6" AB for parking stalls, driveways. Asphalt concrete tests (AC1 through AC 11) locations are shown on Plate B and the test results are tabulated in Table 7. Asphalt concrete was III C3 ''/z" PG 64-10, spread properly using Asphalt Paver and rolled to minimum 95 percent of maximum density. 0 Our Lady Queen of Angels Gymnasium — Final Grading Report Project No.: D-2569-02 E. Laboratory and Field Test Standards Specified compaction control parameters for the project were as follows: 1. Laboratory Test Standard ASTM D-1557 method. 2. Ninety percent minimum compaction for new fill and backfill of subgrade soils. 3. Ninety percent minimum compaction for backfill and trenches. 4. Ninety five percent minimum compaction for aggregate base and asphalt concrete. F. Conclusions and Recommendation Based on field observations and testing, it is concluded that the subgrade preparation, subgrade and aggregate base and asphalt concrete placement, compaction of asphalt concrete parking areas and utility trench backfill covered in this report are accomplished in a satisfactory manner consistent with the project specifications and pertinent recommendations presented in our Geotechnical Investigation Report dated April 12, 2018 and Update Report dated May 26, 2020 and current City of Newport Beach Grading Requirements, and are adequate for their intended uses. In our opinion, grading has been completed in a geotechnically acceptable manner and there would not be an adverse impact (e.g., subsidence, slippage etc.) due to this development on this site or neighboring properties. E. (Motion sensing control functionality) After 15 minutes of vacancy in the outdoor area covered by the motion sensing controls, the installed controls either dim or turn off the lighting. RF. (Motion sensing control functionality) No more than 1500 watts of lighting power is controlled by a single motion sensor. G. (Motion sensing control functionality) Motion sensing controls must be installed for the following luminaires, and may be installed for other outdoor lighting and in combination with other outdoor lighting controls.
