All Rights Reserved 1515 71h St. #129, Santa Monica, CA 90401 www.deckerenv.com 1.0 Executive Summa By request, Decker Environmental performed an Asbestos Survey at the "property address" noted above. The purpose of the survey was to locate and identify suspect Asbestos -Containing Materials (ACMs) and asbestos containing construction materials (ACCMs) of the specified structure(s) that may be impacted by a planned demolition. Personnel performed a visual inspection and subsequent bulk sampling of suspect materials with location and condition of samples documented herein. Richard Perez, a Certified Site Surveillance Technician conducted the sampling and asbestos inspection under the direct supervision of Ryan Decker, CAC. Bulk samples were collected in accordance with USEPA established guidelines document, "Guidance for Controlling Asbestos -Containing Materials in Buildings" (USEPA 560/5-85-024, 1985) and USEPA 40 CFR Part 763 "Asbestos Contain Materials in Schools, Final Rule" (AHERA). After sample collection, samples were delivered to an accredited lab under chain of custody. Building Description: See Appendix B 2.0 Survey Procedures All rooms and accessible areas were entered and visually inspected for suspect materials that may contain asbestos. Condition of the material such as friability, accessibility, activity, and damage of suspect ACMs and ACCMs was assessed and properly documented. Location, type, and quantity was also documented and presented in the Tables below. Each sample is labeled with a unique sample code and documented on a chain of custody for submittal to an accredited laboratory. A written report (survey) is delivered to the client electronically along with lab results and pertinent documentation provided in the Appendices. 3.0 Bulk Sampling Procedures Suspect materials are categorized into homogenous materials; defined as being uniform in texture, color, and date of application. A sampling scheme was then developed based on the suspect material quantities and location. Bulk samples are collected by coring or extracting a small portion of the material and placing in a sealed container with unique sample number for submittal to the lab. Samples are submitted under chain of custody to an accredited lab. Laboratory results can be found in Appendix A of this report. Unless otherwise specified, bulk material samples were analyzed by polarized light microscopy (PLM), EPA Method 600/M4-82-020 and 600/R-93-116. These amounts are not intended for bidding purposes. **This material was further analyzed using a 1000 point count analysis. 6.0 Recommendations Asbestos containing material(s) must be removed by an abatement contractor if the planned project will impact it. Federal and state regulations require that any demolition of asbestos containing building materials be conducted only by workers and contractors who have been properly trained in the correct handling of asbestos. Federal, State, and local statutes specify work practice requirements for demolition activities, and the associated disturbance of ACMs/ACCMs (manufactured material containing greater than 1/10 of 1% asbestos, or 0.1%), as well as the storage and disposal of asbestos -containing waste material. Proper notification, removal techniques, clean-up procedures, and waste storage and disposal requirements are mandated in connection with demolition activities. A mandatory 10-day (for nonemergency asbestos removal) notification is required by the US EPA and local air quality pollution districts prior to the planned removal of 100 square feet or more ACM (greater than 1% asbestos). Current Federal Occupational Safety and Health Administration (OSHA), State (DOSH or Cal/OSHA), and additional local regulatory agencies mandate that ACM/ACCM regardless of quantity, shall be handled during removal, repair, or disturbance by asbestos trained and qualified individuals or contractors. Identified ACMs/ACCMs must also be removed by State licensed Asbestos Abatement Contractors prior to any demolition or construction activities if disturbed or impacted. All asbestos work should proceed under the guidance or direction of an independent State Certified Asbestos Consultant with oversight performed by a State Certified Site Surveillance Technician. At the end of the abatement, Decker Environmental recommends an air clearance inspection be conducted to show that air levels are safe for re-entry. 7.0 Conclusion Upon review of the lab results, the materials listed in Table 4.0 contains asbestos in concentrations greater than 1% and are considered to be asbestos containing materials (ACMs) unless proven otherwise through point count analysis. These specific materials are therefore regulated by the California Occupational Safety and Health Administration (Cal/OSHA), and the South Coast Air Quality Management District (SCAQMD). Contractors and workers must be notified of all ACMs prior to disturbance of the material. A licensed abatement contractor is required to handle, and properly dispose of ACMs if they are going to be removed or disturbed. Materials that are listed as "Assumed" in Table 4.0 are assumed to be ACM by Ryan Decker, CAC and shall be treated as such in compliance with Rule 1403. 8.0 Limitations and Disclaimers This asbestos survey consists of bulk sample collection of suspect materials within the areas specified by the client as outlined in the "Executive Summary". Laboratory results and sample descriptions herein only characterize materials sampled at the date and time of this survey. Hidden, inaccessible, concealed materials, debris, and adverse environmental factors may exist within the subject property, so therefore Decker Environmental cannot guarantee that all suspect materials have been identified and/or sampled. Suspect materials discovered that are not listed in Table 4.0 or 5.0 shall be considered ACM unless otherwise proven through 11 laboratory analysis. Portions of this survey may be deemed invalid if conditions at the subject property have changed since the surveyed date and time. Unless otherwise noted, underground utilities were not surveyed during this inspection. Household items and contents were not sampled and shall be properly removed/recycled prior to any disturbance. Bulk sampling is destructive by nature due to the requirement of an adequate physical bulk piece(s) of material being submitted to the laboratory for the requested analysis. Therefore, Decker Environmental assumes no responsibility for the repair or concealment of sampled materials. Decker Environmental does not assume any liability for the work performed by contractors performing abatement or any type of renovation that may or may not be in conjunction with the results of this survey unless contracted to perform complete oversight of specified abatement/renovation activities.
