Dear Mr. Krotts:
Per your request received on March 11, 2015 (attached), we are assigning the following addresses for the construction of the Birch Medical Project. Suite numbers
will be designated at plan check.
Building 1 20350 Birch Street
Building 2 20360 Birch Street
We will advise City departments; however, it will be your responsibility to inform all other
affected agencies, such as the U. S. Postal Service, Orange County Assessor, utilities, etc. Please be sure to notify your local telephone service carrier so that responses to
any “9-1-1” emergency calls are dispatched to a valid address. The address numbers
should be at least four inches high, of contrasting color to their background, and clearly
visible from the street.

Per our discussion, I would like to formally write to request a change of address for our project
located on Birch Street. As you know, we have acquired 7 parcels along Birch Street with the
following addresses:
20352
20354
20362
20372
20382
20392
20402
20412
Given the fact we will be razing two existing homes and there are several addresses, we would like
to request an address change to eliminate any future confusion for tenants or visitors to our
project.
The proposed addresses are:
Building 1 – 20350
Building 2 – 20360
All of the other addresses can be removed or reassigned.
