ACORD 25 (20091091 The ACORD name and logo are registered marks of ACORD STATEMENT OF AGREEMENT The City of Newport Beach has agreed to retain Rodheim Marketing Group for the public relations functions contained in the attached Newport Harbor Water Quality Phase I Public Relations Budget. The goal of this project is to educate the public on potential hazards to water quality in Newport's harbor and beaches. Where additional services are requested, the agency will provide labor and cost estimates and obtain written approval prior to performing these services. The City of Newport Beach agrees to a $6,900.00 fee for this program to be billed as follows: A good faith deposit of 33% ($2,300.00) will be due upon approval of project agreement; the remaining balance will be divided into two equal monthly installments of $2,300.00. Monthly billing statements will be delivered to the City of Newport Beach by the first of each month and will be due by the tenth of that month. In the event payment is not received within the specified period of time, our services will be terminated until payment is received. All unpaid accounts shall bear interest at the rate of one and one-half percent (1 1/2%). Please indicate your approval by signing the original and one copy. Retain the copy for your files and return the signed original along with a check for the deposit ($2,300). Water Bill Inserts Includes design and artwork for three 1,100.00 two-color 3 1/2 X 6" inserts. Total Projected Costs - Phase I $6,900.00* *Price does not include printing, distri- bution or other out-of-pocket expenses. Recommend printing and distribution through the city.
