Dear Mr. Fracchia:
As you requested, we are hereby adding the address 3731 Fourth Avenue as the
address of the new, rear dwelling' because it faces Fourth Avenue. The other
new dwelling of the two -unit condominium development on the parcel faces
Poppy and shall retain the address 621 Poppy Avenue.
We will notify City departments; however, it is your responsibility to inform any
other affected agencies such as the U. S. Postal Service, Orange County
Treasurer -Tax Collector, utilities, etc. Please be sure to advise your local
telephone service carrier so that responses to any "9-1-1" emergency calls are
dispatched to valid addresses.
The address numbers should be at least four inches high, of contrasting color to
their background, and clearly visible from the street.

Subject: Oasis Senior Center
This is to acknowledge that the address of the Oasis Senior Center is officially changed from 800
Marguerite Avenue to 801 Narcissus Avenue.
I will advise Southern California Edison, Southern California Gas Company, and the U.S. Postal
Service. The other thing that needs to be changed is the Orange County Assessor's data.
Whichever department or person who pays the taxes on that property should notify the County so the
correct situs address appears on the bill. The Assessor Parcel Number is 458-651-02. Please be
sure that the local telephone service carrier has the correct address for emergency responses and for
the City directory.
