As agreed, the specific work that the City would like you to perform as well as the amount of compensation for performing said work are set forth in Attachment 1 to this letter agreement. CHRISTINE CHOE CITY OF NEWPORT BEACH F J4ay °'J$hnsofi', Chief of Police ATTACHMENT 1 TERMS The City of Newport Beach ( "City ") would like to retain the services of Dr. Christine Choe ( "Consultant ") to assist the City with employee support services in connection with the on -duty death of City lifeguard Ben Carlson on July 6, 2014 and with the officer involved shooting on July 11, 2014 at 4603 Seashore Drive. The City and Consultant agree to the following terms: • Consultant shall provide psychological and comprehensive support services to officers and other employees requiring mental health assistance as a result of their involvement with the on -duty death of City lifeguard Ben Carlson on July 6, 2014 and with the officer involved shooting on July 11, 2014 at 4603 Seashore Drive. • City shall pay Consultant for the psychological and support services related to the incidents on July 6 and July 11, 2014 on a time and expense not -to- exceed basis. Consultant's fees for these services are One Hundred Eighty Five Dollars and 001100 ($185.00) per hour. Consultant's compensation for all work performed pursuant to this agreement, including all reimbursable expenses, shall not exceed Fifteen Thousand Dollars and 00/100 ($15,000.00), without prior written authorization from the City. No billing rate changes shall be made without the prior written approval of the City. • Consultant shall submit monthly invoices to City describing the work performed the preceding month. Consultant's bills shall include the name of the person who performed the work, a brief description of the services performed, the date the services were performed, the number of hours spent on all work billed on an hourly basis, and a description of any reimbursable expenses. City shall reimburse Consultant only for those costs or expenses specifically approved in writing in advance by City. City shall pay Consultant no later than thirty (30) calendar days after approval of the monthly invoice by City staff.
